The largest proportion of our investigations arises from fires and explosions. We have handled many thousands of investigations, ranging from small domestic fires to major incidents such as Flixborough (1974), Fort Worth Hilton (1986), Piper Alpha (1988), Angus Chemical Plant (1991), Pemex in Guadalajara (1992), Frontiers Refinery (1992), Alfred P. Murrah Federal Building (1995), Lomac Chemical (2000), Formosa Plastics (2004), BP Oil Refinery (2005), T2 Labs (2007) and Imperial Sugar Refinery (2008). Our services have also been requested at many gas explosions, including Ronan Point (1968) and Putney (1985). These were notable because of the far-reaching implications of their respective causes.
While establishing the root cause of a fire or explosion is normally the primary purpose of an investigation, other issues are frequently highly pertinent. For example, the way in which a fire or explosion was able to develop was crucially important at the Summerland fire on the Isle of Man (1973), Digital (1990), the Sobeys Stores warehouse fire (1993), the Malden Mills explosion and fire (1995), and the South Mimms Service Station (1998).
We have served the marine world for many years, investigating notable maritime casualties such as the Mactra (1969), the Bow Drill III drill rig fire (1987), the Tropical Lion (1989), the Ecstasy Cruise Ship (1998), the SSG Edward Carter (2001), the Hanjin Pennsylvania (2002), the Barge B125 at Port Mobil (2003), the Bow Mariner (2004), and the Star Princess (2006). We have also investigated the causes of materials or mechanical failures which have led to maritime catastrophes, including the Ramsgate Walkway (1994) and the Team Philips racing catamaran (2000).
Our historical links with the aviation world stretch back many years with our scientists and engineers having investigated the causes of many aviation incidents, including major accidents such as Saudi Flight 163 (1980) Air India (1985), Lockerbie (1988), TWA 800 (1996) and BAJ31 (2000), and Air France Flight 358 (2005).
